Complete and mail printed forms
Before completing and printing a form in this section, see if you can accomplish the same thing online at iAccess Web. If you are completing forms in this section of our website, follow these instructions:
- Download Acrobat Reader, if you don't already have a copy. We recommend Acrobat Reader 7 or higher.
- Read the form and any accompanying fact sheet carefully before completing.
- Most forms are interactive. To complete a form while it is on your computer screen, start entering data in the first field at the top left-side of the page. Then press the “Tab” button to go to the next field. Continue this process until you’ve completed the form.
- Print a form by clicking on the Acrobat printer icon or click on “File” then select “Print” and click “OK”. Make sure an extra copy for your own records.
- Once you’ve printed a form, you may wish to verify the information. Make sure you’ve included your social insurance number (SIN) and your name.
- Check the bottom of each page to ensure you’ve included the date and your signature.
- Attach any documents requested.
- Return the form and any accompanying documents to us by fax or mail.
