Getting started:
  • Download Acrobat Reader, if you don't already have a copy.

    (We recommend Acrobat Reader 4.0 or a more recent version.)
Complete forms online:

To make it easier for you, we’ve added some features that will make it possible to complete the form while it is on your screen. Then all you need to do is print and sign it . You can also choose to print forms and enter your information by hand.

  • Read the form carefully – it may have special instructions.
  • Start by entering data in the first field at the top left-hand side of the page, and then just press the “Tab” button to go to the next field. Continue this process until you’ve completed the form.
  • Print a form by clicking on the Acrobat printer icon or click on “File” then select “Print” and click ok.
  • Make sure to print a copy to keep for your own records.
  • Remember to include your signature and date.
  • Return it to us by fax or mail.
Returning completed forms:
  • Once you’ve printed a form, you may wish to verify the information. Make sure you’ve included your social insurance number (SIN) and your name.
  • Check the bottom of each page to ensure you’ve included the date and your signature.
  • Make sure you’ve attached any other documents requested.
